We accept most major insurance plans. Please call our office at (979) 703-6112 to confirm that your specific plan is accepted prior to scheduling your appointment.

It is your responsibility to understand your insurance benefits, including co-pays, deductibles, and any referral requirements. Some insurance plans require a referral from your primary care physician before seeing a specialist — please check with your plan before your visit.  Notify our office immediately with any changes to insurance.

New Patient Information

We welcome new patients of all ages. To schedule a new patient appointment, please call (979) 703-6112. Our staff will collect basic information and explain what to expect at your first visit.

Please arrive with New Patient packet completed.

Please bring to your first appointment:

• Photo ID
• Insurance card(s)
• List of current medications (including over-the-counter medications and supplements)
• Records of prior allergy testing or relevant medical records, if available
